Application for maternity benefits from the health insurance provider

During the maternity leave periods before and after childbirth, as a member of a statutory health insurance plan, you are entitled to benefits under maternity leave law. 

Where do you submit the application?

• With your statutory health insurance provider

When should you submit the application?

• Before the start of the maternity leave period (6 weeks before the estimated due date)

You will need the following documents to submit the application:

• “Certificate of the estimated date of delivery” (certification from the gynecologist regarding the pregnancy and due date)

• Health insurance form (often available on the digital insurance portal) or an informal letter
